Levels of care
Levels of care provided in the following parishes by A Care Partner:
1.
Companionship
Companion Care can be used not only in a client’s home, but also in an Assisted Living facility, Skilled-Nursing facility, Home Care, or even in a hospital. An example of a common use of Companion Care can be to provide monitoring or supervision with the aide of fall prevention to avoid injury or setbacks in the recovery of a previous injury. These care services are also ideal for a person who is transitioning back into their home or community after being discharged from a hospital stay.
2.
Homemaker Care
Homemaker Care is not confined to the home. Homemaker assistance may include light housekeeping, laundry, meal preparation, and transportation. We also provide transportation for our clients who have lost their ability to drive themselves, or those who have safety concerns about their commute. Along with providing transportation, we can escort our clients and keep them company for their personal obligations, such as doctor appointments, physical therapy treatments, shopping, religious service, or special events.
3.
Personal Care
Personal Care Assistance is a service to individuals in their own homes and communities, who need assistance caring for themselves as a result of old age, sickness, disability and/or other inflictions. Caregivers may provide personal care assistance to someone in needed that includes assistance with the activities of daily living (ambulation, transferring, bathing, dressing, toileting, medication reminders, hygiene, etc.), as well as items listed in Companionship and Homemaker Care.
